According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, three out of five stars for staffing issues, and four out of five stars for quality measures.

The state investigators determined that the nursing home “failed to ensure [two residents] received a complete assessment to identify fall hazards and the necessary interventions that were developed to prevent falls.” One resident’s “fall risk was not reviewed after a fall intervention to prevent recurrent falls [and] were not individualized or implemented.
These “failures resulted in the resident’s recurrent fall from the bed leading to hospitalization after the Nephrostomy (a tube placed through a hole in the skin that extends into the kidney, a by the organ, which filters blood and produces urine) tube was displaced as a result of the fall.”
The resident’s “fall interventions were not implemented as indicated on the Plan of Care and were not revised timely after a fall. These failures placed [the resident] at risk for recurrent falls.”
The facility “failed to ensure [one resident] received the necessary treatment and services to promote healing of moisture associated skin damage (MASD) (skin breakdown caused by prolonged exposure to a source of moisture such as urine, stool, sweat, wound drainage) when the Licensed Nurses did not obtain Physician ordered treatments for the right buttocks for three days and for the left buttocks for seven days.”
The “failure resulted in the delay of the healing of the MASD to the right and left buttock of [the resident].” A family member stated that the resident “had a sore on her bottom [and] was getting ointment applied to help heal it.”
A Licensed Nurse stated that the resident “had delays in wound healing because there was no evaluation to renew or extend the treatment of the MASD on [their] right buttock.” The nurse stated that they did not follow protocol, and the resident did not receive treatment promptly. The actions of the nursing staff violated the facility’s policy and procedure titled: Skin Assessments from 2015, that reads in part:
“… to ensure that every resident admitted to and residing at the facility will have a skin check on a routine basis for any conditions that require medical interventions. If skin breakdown occurs, a resident will receive the appropriate treatment per Physician’s orders.”
