The state investigators determined the nursing home "failed to ensure [two residents] were safely assisted during standing and transferring. The facility failed to ensure [one resident] was transferred from the bed to the wheelchair with two-person assistance. This [failure] had the potential for [the resident] to be injured.”
In the other incident, the facility failed to ensure that the second resident “remained free from accident hazards [when the resident] was observed standing and holding onto a wobbly footboard frame on his bed while two Certified Nursing Assistants (CNAs) were changing his incontinence brief. This failure had the potential to contribute to the resident’s falls and injuries.”
The facility “failed to ensure [one resident] was provided with adequate pain assessment and management. This [failure] had the potential for the resident’s pain not being treated effectively.”
A Licensed Vocational Nurse indicated that the resident “was sometimes able to verbalize her pain with episodes of moaning.” The Licensed Vocational Nurse stated that the resident “complained of back pain most of the time [and said that] she would offer non-pharmacological interventions first and assess the resident after.” The nurse stated that “she used the pain scale to assess the resident’s pain.”
The state investigators reviewed the resident’s Medication Administration Record that revealed that on six separate occasions between February 20, 2019, and March 20, 2019, the resident had reported a pain level of seven or higher out of ten. The Director of Nursing was informed of the level of pain documented in the medical records and asked, “what the licensed nurse should do if the resident’s pain level was above the parameters of the medications prescribed by the Physician.”
The Director of Nursing responded that “the licensed nurse should have informed the Physician that the resident’s pain was not being controlled with the two current pain medications.”
