  • Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Prevent Avoidable Accidents - citation date October 18, 2018

  • According to surveyors, the "facility failed to provide the level of assistance required during activities of daily living (ADL) care resulting in a fall for one resident." The resident "was assessed as needing extensive assistance with bed mobility, dressing, toilet use, and personal hygiene, and total assistance with transfers "using "two plus people with bed mobility, transfers, and toilet use."

    The survey team reviewed the resident's Electronic Medical Record that revealed the resident was "totally dependent on staff or turning and positioning in bed and require the assistance of two staff members to reposition and turn in bed." However, the Facility Incident Report documented that the Charge Nurse observed the resident "on the floor in her room with a Certified Nursing Assistant (CNA) at her side. The resident was assessed and assisted back to bed by two members of the staff using a Hoyer lift."

    The CNA documented the event in a written statement saying that she was in the resident's room between 9:30 PM and 10:00 PM on the night of the incident" and was getting ready to change her. The CNA further reported that she went to the closet to get a brief and while at the closet, she observed the resident's left leg slighting over the other. The CNA hurriedly went to the resident's assistance, but by the time she arrived at the bedside, the resident's legs were both over the side of the bed, the resident was holding onto the side rail which made it difficult for the CNA to return her legs to the bed."

  • Failure to Provide Necessary Care and Services to Maintain the Resident's Highest Well-Being - citation date October 13, 2016

  • The nursing home "failed to provide prescribed medications to ensure adequate pain management for [one resident] that experience mild to severe pain after hip replacement surgery." The surveyors say that the inadequate management of the resident's "pain medication regime that included [pain killers including Tylenol] caused the resident to experience pain, causing actual harm identified to have existed beginning on September 22, 2016."

    Documentation revealed twenty-two pain medication dosing opportunities that were missed between September 21, 2016, and October 5, 2016. The Medication Error Report dated October 5, 2016, revealed that the facility "failed to retain a written prescription" for the resident's pain medications until the last day. Documentation shows that the pain medication was not administered to the resident "despite the signatures of nurses on six occasions because the medication was not available in the facility."
