According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, three out of five stars for staffing issues, and four out of five stars for quality measures.

The state investigators determined that the nursing home failed to ensure to residents “were free from abuse perpetrated by a visitor at the facility, who was also the spouse of a resident residing in the facility.”
Documentation shows that on February 11, 2019, “without the consent of [the resident], the visitor came up behind [the resident] and placed his hands down the shirt of the resident, while the resident sat in the Dining.” The resident stated he/she “was scared and shocked by the incident.”
Due to the facility not implementing interventions to protect or prevent further abuse from occurring, on April 29, 2019, “without the consent [the same visitor kissed a second resident] on the forehead and ran his hand down the resident’s clothing to the left breast and to the resident’s pubic area.”
At the time, this resident “was hitting and screaming at the visitor to stop. A staff member intervened and instructed the visitor he had to leave the facility immediately.” This resident stated he/she “cried as he/she felt violated.”
The investigative team determined that these deficient practices placed both of these residents “reviewed for abuse in Immediate Jeopardy of serious injury, harm, impairment, or death.”
According to the Alabama Department of Public Health Online Incident Reporting System, “on February 14, 2019, at 4:58 PM, the facility reported an allegation of sexual abuse that involved [a resident and another resident’s spouse] visitor. The report documented the incident occurred on February 11, 2019, at 11:30 AM, in which the Unit Manager received a call at 3:44 PM from the resident’s representative (the resident’s spouse).”
The resident spouse stated that the resident contacted him one day this week and stated, “that someone put their hands down [the resident’s] shirt.” The spouse said that while the resident was in the dining room, “a male came up from behind [the resident] and put his hand under his/her shirt.”
