  • Failure to Provide Residents Proper Treatment to Prevent the Development of New Bedsores or Allow Existing Pressure Wound to Heal – citation #F314 date December 30, 2016
  • The state investigators determined that the nursing home failed to ensure that one resident reviewed “received the proper care and services to promote healing of a pressure ulcer.”

    The state investigators observed a resident “receiving care by two Certified Nursing Aides after incontinent of bowel movement.” During the observation, the surveyor saw the resident’s backside that “revealed a round open wound about the size of the Silver Dollar on the coccyx (upper part of the buttock).” The wound was open, without any type of dressing. The surveyor left to allow the staff to finish cleaning up the resident.”

    Less than two hours later, the resident was then observed in the presence of the Assistant Director of Nursing who “removed the resident’s adult brief and noted the wound remained without a wound dressing.” The Assistant Director “was unaware that the wound dressing had come off and was removed earlier in the day.”

    The Assistant Director stated that “she had seen the Wound Care Nurse outside the resident’s door earlier with some wound care supplies. The Assistant Director of Nursing agreed that the aides should have notified a nurse that the dressing had come off.”

  • Failure to Ensure That Residents Are Safe from Serious Medication Errors – citation #F333 date October 12, 2017
  • The facility “failed to prevent potential significant medication errors related to duplicating insulin administration to [one resident] observed during the medication observation passes.” This deficient practice involved one resident “observed for insulin administration observed during the medication observation pass.”

    A Registered Nurse (RN) working the morning shift was not sure if insulin had been administered to the resident by other staff members that morning. Even so, the RN “continued to withdraw both insulins to be administered without checking with [the other staff member] first to substantiate whether the insulin was administered or not.”
