  • Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Prevent Avoidable Accidents – citation #F689 date November 21, 2018
  • The state investigators determined the nursing home "failed to develop and implement resident-specific and relevant interventions after a fall incident.” Additionally, the nursing home “failed to complete neuro checks after two unwitnessed falls and failed to provide adequate supervision for [one resident].” These deficient practices “resulted in repeated falls and placed the resident at risk for injury or harm.”

    The state investigators reviewed the resident’s medical records and Fall Scene Investigation Report that indicated that just after noon, the resident “was found on the floor in a lying prone position.” The report indicates that the resident “was trying to move from the bed to the chair, lost balance, and slid on the floor.” At that time, the resident was “alone and unattended.”

    A report indicates that the resident “has impaired vision, strays off the street path of walking, changes gait patterns when walking through doorways, has lurching, swaying, or a slapping gait and has decreased muscle coordination.” The resident is also “at moderate risk for falls.”

    A review of the medical records indicated that after the resident had fallen, there was “no documented evidence that a neuro check was performed.”

  • Failure to Provide a Safe, Appropriate Pain Management Plan of Care for a Resident Who Requires Those Services – citation #F697 date November 21, 2018
  • The “facility failed to provide interventions timely, failed to document interventions implemented to address the pain, and failed to update the Plan of Care, including interventions to address the resident’s complaints of pain after a fall incident.”

    A review of the resident’s medical records indicate that the resident documented above had fallen and complained of pain in the left shoulder upon movement. The nursing staff had been directed to administer a dose of pain-relieving medication and used ice packs. However, a review of the clinical records indicates that “there was no documentation the pain medication was offered and administered to relieve the resident’s pain, or any interventions provided at the time of the fall to address the resident’s left shoulder pain.”

    Additionally, the Care Plan was “not updated and did not reflect specific approaches to address or minimize the resident’s pain.”
