The nursing home "failed to report two allegations of misappropriation of resident funds to the State Survey and Certification Agency."
The surveyors reviewed the Loss Items Report that revealed "that the resident reported that $50 was missing from his room. Review of the results of the search revealed the resident's room was searched and a $10 bill was found in his jacket hanging in the closet. The resident was educated about facility policy and that money could be locked away."
The surveyors interviewed the Administrator who stated "that the missing money was not reported to the State Survey Agency because they found the resident's $10. When asked if the facility found the remaining $40, the Administrator stated it was not found."
According to investigators, the facility "failed to ensure one resident with existing pressure ulcers was turned and repositioned every two hours to prevent the development of additional pressure ulcers."
The state surveyors observed the resident at multiple times between 8:38 AM and 4:30 PM "lying on her left side on top of an air mattress overlay on the bed. The head of the bed was at a 45-degree angle" while her feeding tube was infusing.
Cap the resident "was clean, neat and without odors [and the] staff placed a pillow between her knees and feet. However, the staff failed to turn and reposition the resident at those times to prevent the development of additional pressure ulcers."
The facility "failed to ensure that a Broda chair for [a cognitively impaired] resident was maintained in a safe manner." The surveyors observed the resident while "he was sitting in his Broda chair in the hallway. The front part of the patient's right arm of the Broda chair was missing, and a bolt was observed sticking straight up from the frame. The resident's right hand was observed resting on the padded right arm of the Broda chair."
The Maintenance Supervisor stated that "he had not been notified of the exposed bolt until several hours ago." The supervisor said that "staff should monitor resident equipment for any maintenance issues and notify him of any accident hazards right away."
