  • The state investigators determined the nursing home "failed to implement its Abuse Prevention and Prohibition Policy by conducting a background screening check before [a Certified Nursing Assistant (CNA)] began working in the facility.

    The state investigators reviewed the personnel file of a CNA. However, “there was no documented evidence that indicated a background screening check was conducted before the employee began working in the facility,” in violation of state and federal laws.

  • Failure to Respond Appropriately to All Alleged Violations – citation #F610 date November 22, 2017
  • The facility “failed to have evidence that all alleged violations of mistreatment were thoroughly investigated and failed to prevent further potential mistreatment while the investigation was in progress as indicated in the facility’s Abuse Investigation policy.”

    The state investigators reviewed the resident’s Complaint Intake Form provided by the Department of Health. The documentation shows that a family member had specific concerns, including that the resident stated that “she was left on the bedpan.” The resident said that “the nurse did not come back to take the resident off of the bedpan and she was sitting in feces.”

    As a result of a lack of care, the resident called the family member at 7:30 PM and “asked for the family member to get help.” At that time, the Charge Nurse assisted the resident “off of the bedpan.”

  • Failure to Ensure the Nursing Home Area Remained Free of Accident Hazards and Risks and Provides Supervision to Prevent Avoidable Accidents – citation #F323 date January 2, 2018
  • The nursing home “failed to check the temperature of the hot goal soup being served to [one resident]. This deficient practice resulted in [the resident] spilling hot soup on the resident’s thighs and groin area.”

    The nursing staff transferred the resident “to the Acute Care Hospital where treatment was provided for the resident by making an incision in a blister located on the resident’s right inner thigh area.” As a result, the investigator’s made an unannounced visit to the facility “to conduct a complaint investigation regarding a burn that occurred involving [the injured resident].”
