  • Failure to Respond Appropriately to All Alleged Violations – citation #F610 date November 20, 2018
  • The state investigators determined the nursing home "failed to ensure staff had followed their abuse prevention reporting policy when [one resident] alleged he was mistreated by a staff member. The facility did not interview or suspend the potential staff perpetrator, did not identify or interview potential witnesses, and did not report the allegation to the ombudsman, the police or the California Department of Public Health.”

    The state investigators determined that “this failure placed residents at risk for further mistreatment if the allegation was proven.” A review of a resident’s Progress Note indicated that the resident “left the facility [unsupervised around] noon and the staff found him and brought him back to the facility.”

    The resident “verbalized at that time he did not want to stay in the facility anymore [and] signed the Release of Responsibility for Discharge Against Medical Advice on the same day.” The resident stated in a social service note that before “leaving the facility, a male nurse woke him up during the night to give him pain medication and then the male nurses hurt his stomach.” A Licensed Vocational Nurse stated that the resident “had told her a male staff member at night had hurt his abdomen.”

    A Grievance/Complaint Report indicated “staff did not identify or interview potential perpetrators or witnesses, did not suspend potential staff members and the facility did not report the allegation to the ombudsman, police, and the Department.” The Director of Nursing confirmed that “she did not interview any staff and did not report the allegation to any local agency.”

  • Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Prevent Avoidable Accidents – citation #F689 date February 6, 2019
  • The “facility failed to provide physical assistance during a transfer to prevent a fall for [one resident who] was unassisted. The pad alarm was sounding and [the resident] fell on the floor while getting up from bed on January 7, 2019.” As a result of the fall, the resident suffered injuries that required sutures.
