The state investigators determined that the nursing home “failed to prevent a major injury which resulted from a fall. The resident sustained [injuries].” This “practice has the potential to affect all 166 residents present in the facility at the time of the survey.”
A review of the resident’s MDS (Minimum Data Set) Discharge/Return Anticipated Assessment dated March 24, 2018, indicated that the resident had “no cognitive impairment, [but] requires extensive assist with 2+ person physical assist for transfer, toilet use, and the resident had one fall [without] a major injury.” However, additional information recorded that the resident “had no falls in the last ninety days.”
The resident’s Fall Care Plan documents that the resident “does not call for assistance despite encouragement to ask for help. Poor safety awareness [with a goal to] minimize the risk of an additional fall and fall-related injuries from a preventable fall through the next review date.” However, review of the Care Plan and its updates and revisions show that there was no documentation “to assist in preventing falls that could potentially result in a fracture.”
A nursing assistant informed the nurse that she had “noted the resident on the floor, upon arrival. The resident was noted on the floor in a sitting position.” The document says that the resident had stated, “I fell on the floor while trying to sit on the toilet. The resident is alert and verbally responsive, denied pain at this time, respite duration was even and long labored.”
Four days later on March 24, 2018, the resident began complaining of pain of the right leg that was tender to the touch. The nursing staff provided Tylenol and ordered an x-ray. A review of the resident’s Medication Administration Record indicated that the facility staff “was to monitor pain every shift.” The lab results indicated the resident did suffer an injury involving “acute right femoral right neck fracture.”
