The state of California and Medicare.com routinely update their comprehensive list of deficiencies online to reflect all violations.

According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, three out of five stars for staffing issues and three out of five stars for quality measures.

  • Failure to Timely Report Suspected Abuse, Neglect or Theft and Report the Results of the Investigation to Proper Authorities – citation #F609 date March 7, 2019
  • The state investigators determined the nursing home "failed to ensure an allegation of employee-to-resident abuse was reported to the California Department of Public Health (CDPH) within twenty-four hours.” This “deficient practice resulted in a delay in CDPH being aware of the incident and a delay in investigating the facts related to the abuse allegations.”

    A review of a resident’s medical records indicated that the resident claimed that morning that a Residential Care Specialist “had grabbed her hard while getting her ready that morning around 8:30 AM. According to the head to toe skin checks on the SBAR (Situation, Background, Assessment, Recommendation) Communication Form and Progress Note, [the resident] had a right middle finger skin discoloration.”

    The Five-Day Summary Report indicated that the resident had made an allegation that a Certified Nursing Assistant/Residential Care Specialist roughly handled her. However, the CDPH did not receive a completed form involving suspected incidents of abuse or neglect from the facility.

    In a separate summary statement of deficiencies dated July 2, 2019, the facility “failed to report to the Department of Public Health an alleged abuse incident in a timely manner for [one resident].” This “deficient practice had the potential to result in under-reporting of alleged cases of resident abuse and failing to investigate resident abuse in a timely manner.”

    The investigation involved a severely cognitively impaired resident who “has the ability to make themselves understood and the ability to understand others.” The resident’s Suspected Abuse Investigation Form indicated that the resident “alleged a Certified Nursing Assistant grabbed her arm.” The report indicates the CNA “grabbed her right arm” while changing the resident’s incontinence brief. The resident resisted in the middle of the process and the CNA admitted that “she grabbed the resident’s arm only to prevent her from falling.”

    The CNA stated that “she couldn’t get help on time because it happened so fast. Multiple residents and staff were interviewed and none said anything negative about the CNA. There were no witnesses to the event. There was no previous complaint of abuse or mistreatment from the CNA.”
