According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, three out of five stars for staffing issues, and three out of five stars for quality measures.

The state investigators determined that the nursing home “failed to ensure that supervision was provided to prevent future falls for [one resident].” In one case, a resident had two witnessed falls within 35 days (between May 9, 2019, and June 13, 2019), with “one fall involving unsupervised outdoor use of a motorized wheelchair.”
The resident “was observed operating his electric wheelchair in the parking lot after recommendations from a Rehabilitative Therapy evaluation included supervision while on the sidewalk and use of the motorized wheelchair in only the patio and outdoor sitting areas.”
At 8:30 AM on July 9, 2019, the resident, while in their electric wheelchair was “moving toward parked cars in one of the facility’s driveways. He was moving at a quick pace. The facility Receptionist, Nursing Home Administrator, and Director of Nursing were informed that the resident was near parked cars at the end of the driveway in the parking lot.”
At that time, the Nursing Home Administrator stated “he does go outside, he has been screened for safety. He knows he is not supposed to go beyond a certain point. We will take care of it.” By 9:15 AM, the resident was again observed, “outside in his wheelchair, moving around and near parked cars, several feet from the entrance to the facility.”
A subsequent Progress Note revealed that the resident “fell outside of the facility; out of his wheelchair.” The note stated a “visitor was driving by and saw him and proceeded to call 911.” Notations indicated that “the ambulance was on the scene when the facility staff was notified. The nurse was able to see the resident lying on his right side and an abrasion to his left forearm. He was complaining of right hip pain.”
The “emergency department nurse notified the facility that the resident did not have any fractures. Follow-up Progress Notes on May 10, 2019, revealed that the resident stated [that] he was in his wheelchair going down the sidewalk in the front of the building to sit by the doors outside at the corner of the building.”
