According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, three out of five stars for staffing issues and two out of five stars for quality measures.

  • Failure to Protect Every Resident from All Forms of Abuse Including Physical and Sexual Assault, Physical Punishment, and Neglect by Anybody – citation #F600 state February 2, 2018
  • The state investigators determined that “the facility failed to ensure that [one resident] was free from verbal and mental abuse.” The incident involved a moderately, cognitively impaired resident whose MDS (Minimum Data Set) Assessment “also included that the resident had adequate hearing, usually made her self understood and understood others.”

    A review of a Nurse’s Note indicated that “a family member had visited the resident after dinner in the dining room and had yelled at her and shook his finger in front of her, telling her he did not appreciate her making him drive over 400 miles to come to see her.” Later, “the family member was observed to assist the resident into the common area and then exited the facility.”

    The note indicates that the Certified Nursing Assistant (CNA) “informed the nurse about the resident after the family member left the building.” A facility investigative report revealed that at 6:00 PM, “a family member who was visiting the resident in the dining room became frustrated with the resident because she was not eating. A visitor observed that the family member of [the resident] push the resident’s head back with his finger.”

    A visitor present in the dining room stated that the resident was very loud and “telling the resident she could not go home with them. The visitor reported that this is not the first time I have witnessed this.”

    A second visitor stated that the resident’s family member “pushed the resident’s head back with his finger and said ‘you are pissing me off.’ The statement included the lady looked very afraid.” A third visitor stated that the resident’s family member had yelled at the resident stating “you piss me off, this is the same ‘sh**’ you did when you were at home, and I am not going to put up with this ‘sh**.’”

    This visitor stated that the resident “used his right index finger and pointed it on the resident’s forehead and stood over her in anger. Then the family member wheeled the resident out of the dining room.”
