  • Failure to Develop, Implement and Enforce Policies That Prevent Mistreatment, Neglect or Abuse of Residents – citation #F226 date September 29, 2017
  • The state investigators determined that the nursing home “failed to operationalize their abuse prevention policy by failing to perform complete employee background checks as prescribed by the Department’s Health Care Worker Registry for twelve of thirteen employees reviewed.”

    A review of thirteen employee files from the Director of Nursing, Certified Nursing Assistants, Registered Nurses, housekeepers and dietary aides failed to include documentation “that the facility searched the required six websites” that include HHS, Illinois Sex Offenders Registry, Illinois Department of Corrections Sex Registrant, Illinois Department of Corrections Inmate Search, Illinois Department of Corrections Wanted Fugitives, and National Sex Offender Public Registry.

  • Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Prevent Avoidable Accidents – citation #F689 date September 26, 2018
  • The nursing home “failed to ensure safe transfers to prevent injury for [one cognitively intact resident].” A Case Information Note documented by an orthopedic surgeon documents “infection of Left Total Knee Replacement.”

    A Physical Therapy Assistant documented there was increased assistance from the assistant because the resident was “more fatigued [and] does not feel well and has been having episodes of diarrhea.” The Assistant stated that the resident “does appear to be pale in color [and that nursing was aware of the resident’s] complaints.”

    A review of the resident’s SBAR (Situation, Background, Assessment, Recommendation) documents a “rotation of the left lower extremity, pain in the left lower extremity, heard a pop this morning with the transfer. This document also states [the resident] was assessed, the doctor was notified, and [the resident] was sent to the emergency room.”

    The resident stated that in the early morning hours around 3:00 AM, two Certified Nursing Aides had used a pivot board to “move me from the bed to the wheelchair.” The resident stated, “my bad left leg got tangled up with my good leg, and I felt a pop and a lot of pain.”

    The nursing staff continued to take the resident to their medical treatment. During that time the resident stated “I was miserable. Finally, when I got back from [my treatment] they sent me to the hospital. I was not feeling good that day anyway. I had diarrhea and was weak.”
