  • Failure to Protect Every Resident from All Forms of Abuse Including and Physical and Mental Abuse, Sexual Assault, Physical Punishment and Neglect by Anybody – citation date April 11, 2018
  • According to surveyors, the “facility failed to ensure that residents were free from neglect, indicated by [one resident being transferred] to the facility from an acute care facility in the afternoon. By that evening, the resident complained of nausea; the resident was later found to be in bed unresponsive and later passed away, less than six hours of being in the facility.”

    “It was later discovered that the day shift Registered Nurse (RN) and the evening shift RN were not made aware that the resident was in the room.”

  • Failure to Ensure Residents Receive Proper Treatment to Prevent New Bedsores or Heal Existing Pressure Sores – citation date February 1, 2018
  • The nursing home “failed to ensure that [three residents] review for pressure ulcers received the necessary treatment and services to promote healing and prevent new ulcers from developing.” The nursing home also failed “by not conducting consistent pressure ulcer wound assessments in ensuring affect the prevention measures were implemented [for three residents].”

    This deficient practice “resulted in bilateral (both sides) heal and sacral wound’s developing, wound worsening due to inappropriate interventions being performed and revised as resident conditions warranted, and a coccyx (tailbone) and sacral wounds were not being recognized in the facility.”

    The investigative team interviewed the MDS coordinator That who confirmed that “there was no documentation in the electronic records that the wound care nurse documented a skin observation and documented and intervention.”

    The surveyors interviewed the Assistant Director of Nursing who said “we do not have a Wound Care Nurse right now. The Wound Care Nurse never did all the treatments. She was responsible [for making] sure all the treatment orders were in place.”
