The nursing home “failed to identify and divide timely care and services required for [seven residents] noted is not receiving the need for professional services to advance or maintain the functional status.”
In one incident, a resident “requires had support to assist in holding her head in a comfortable position, causing pain to the resident.” Another resident’s “oxygen tubing was too long and getting caught under the bed wheel, her feeding tube [was being] turned off, and a Certified Nursing Assistant (CNA) for getting the turned it back down.”
A third resident was not given restorative services, and a fourth resident was not identified as having restorative needs and having those needs provided promptly.
Other residents in the facility were identified as “not having adult briefs (disposable undergarments) available and waiting to be changed. These deficient practices could result in any resident in need of assistance to experience a decline in their functional abilities, being at risk for skin breakdown, and having pain due to being uncomfortable.”
According to investigators, “the facility failed to ensure the grievances were resolved for [six residents] noted to have expressed concerns about laundry services at the facility. This deficient practice has the potential to result in feelings of frustration for residents due to the failure to obtain resolutions of concerns.”
The nursing home “failed to consistently provide cares and services to prevent the development of avoidable pressure ulcers for [six residents] that were reviewed for facility-acquired pressure ulcers.”
The residents were identified by the facility’s Wound Care Certified Nurse as “having acquired pressure ulcers at the facility. The deficient practice of not addressing resident risk factors for pressure ulcers preemptively resulted in harm to the residents.”
The nursing home failed to ensure the two residents “noted to be interested in therapy to improve or control urinary incontinence or educated on potential interventions available and provided options.” The deficient practice “may result in a resident of avoiding social situations, have been moisture associated skin damage and a failure to gain a maximum level of comfort and happiness in life.”
