    The facility Administrator stated that the resident “had been moved to a different station, away from the residents who knew him. The other residents who knew him would see him and were cautious.” One of the residents assaulted by the injured patient stated that the patient “had punched me in the right upper arm. She stated he had pulled her ponytail and previously hit her.” The injured resident stated that “I am afraid of him, he’s still loose.”

    The injured resident stated that “the facility would have a coffee social in the dining room in the mornings and [the aggressive resident] would always be there, and she likes to go to the dining room for the activities too, but was afraid of him.” The investigators interviewed the aggressive resident who said through an interpreter “he would go to the coffee socials.”

    A facility sitter stated that “she was walking in the hall with [the aggressive resident] on August 26, 2019 when they passed [the other assaulted resident] sitting in his wheelchair in the hall.” The sitter stated that “the [second injured resident] gave the [aggressive resident] a look, indicated by her head lowered a little bit and eyebrow slightly down (furrowed), to which [the aggressive resident] put up his fists in a gesture.”

    The facility sitter said that “after he did something wrong, she would correct him, not before to stop him” from causing injury and fear in other residents.
