Dover Delaware Nursing Home Safety Concerns Two Stars Rating

The state of Delaware and the federal government regularly updates its long-term care home database system with complete details of all deficiencies, citations, and violations.

According to Medicare, this facility maintains an overall rating of two out of five stars, including two out of five stars concerning health inspections, four out of five stars for staffing issues and three out of five stars for quality measures.

  • Failure to Report and Investigate Any Act or Reports of Abuse, Neglect or Mistreatment of Residents – citation #F225 date October 13, 2016
  • The state investigators determined that the “facility failed to report, thoroughly investigate an allegation of abuse/mistreatment, and failed to follow the facility’s Abuse Policy.” A review of the Grievance and Concern Form showed that the resident had complained to the Director of Social Services that he/she would like a different Certified Nursing Assistant (CNA).”

    The resident stated that the CNA he/she has providing care most days “during the day shift talks nasty to the residents, and he/she is tired of being yelled at.” The resident “goes on to state that one day, the CNA slammed down a meal tray on the table and [the resident] was afraid of the CNA.”

    Documentation indicates that the resident “repeated several times that he/she was afraid of the CNA.” A subsequent entry note written by the Nursing Home Administrator reflected that the Unit Manager, Registered Nurse (RN), stated that they were with the CNA in the resident’s room “and no items were being thrown by [the CNA].”

    The Administrator further documented that the resident “stated that he/she was fine, food is good, the staff is fine, and that the telephone does not work.” The Administrator discussed with the resident that the CNA “was trying to encourage his/her independence.”

    However, further documentation of the same day indicates that the Administrator had received a call from the local ombudsman “who had gotten a call concerning the allegation.” The Administrator documented that they went over the allegation and “investigation with the ombudsman and documented that [the resident] also had [a medical condition].”

    Subsequent Nurse’s Notes indicated that the resident was accusing staff with throwing stuff in their room and that “no inappropriate comments/suggestions were made, nothing was thrown or put down in a way that could be misinterpreted as being thrown.”
