  • Failure to Immediately Notify the Resident, the Resident’s Doctor or Family Members of a Change in the Resident’s Condition Including a Decline in Their Health or Injury – citation date July 19, 2018
  • According to investigators, the facility “failed to notify the Physician regarding a change in medication.” The deficiency involved one resident who uses an IV bag for the administration of antibiotic medication to treat a surgical wound. The surveyors reviewed the medication bag label that administers the medication at a dosage that was not according to the Physician’s orders.

    During a review of the resident’s doctor’s orders, the resident was to receive an infused medication at 100 mL “into the catheter every four hours” to treat “bacteria in the blood.” However, during an interview with the pharmacist, it was revealed that “the pharmacy received verbal approval from [the Unit Manager] to change the medication because the pharmacy was extremely low on 100 mL bags.”

    The pharmacy recommended a change in the infusion rate of antibiotics but stated “the pharmacist would not notify the Physician of the change as that was not their typical protocol. The pharmacy notified the facility and the facility was supposed to get the clarification order when changing anything on order.”

    The surveyors documented that the facility “failed to ensure services provided for [a resident] met professional standards of quality as outlined in the care plan. The staff administered [the resident’s] intravenous antibiotic at a higher concentration then what the Physician ordered.”

  • Failure to Provide Care for Residents That Builds or Maintains Their Dignity and Respect of Individuality – citation date June 8, 2017
  • The state investigators “determined the facility failed to promote care for residents [that] maintained or enhanced each resident’s dignity and respect.” In one incident, a resident’s “urine drainage bags were not in dignity bags and visible from the hallway.” The second resident “reported that the staff in [their] room had loud personal conversations with no recognition or regard for the resident.” A third resident “was observed in the dining room with no meal tray for twenty-five minutes after the staff served all the other residents.
