  • The investigators determined the facility “failed to ensure individuals who reported allegations of abuse were not retaliated against.” The incident involved one resident involved in an abuse investigation.

    The event was documented and reviewed revealing that a housekeeper “reported to the facility staff that while she was cleaning [a resident’s] restroom, she heard a Certified Nursing Assistant (CNA) verbally abuse [the resident] by saying ‘do not hit me b--ch, turn your stinking a-s over.” During an interview with the allegedly abusive Certified Nursing Assistant, the CNA “did not recall any incident [with that resident].”

  • Failure to Ensure Residents Receive Proper Treatment to Prevent New Bedsores or Heal Existing Pressure Sores – citation date September 13, 2018
  • According to investigators, the facility “failed to provide care and services to prevent pressure ulcers for [one resident].” The surveyors observed the resident’s “heels resting on a mattress surface of the bed.” The resident’s records revealed that “the facility assessed the resident was at risk for developing pressure ulcers and requires extensive assistance of two staff members for bed mobility.”

    During an interview with the Certified Nursing Assistant (CNA), it was revealed that the resident’s “heels were not positioned properly on the bed as they should be off-loaded from the surface [to] prevent a pressure ulcer.”

  • Failure to Provide Adequate Nursing Staff Every Day to Meet the Needs of Every Resident – citation date September 13, 2018
  • The state surveyors determined “the facility failed to provide sufficient nursing staff [to] provide services that meet resident’s needs.” The deficiency involved eight residents.

    One resident at the facility was interviewed and stated that it took Certified Nursing Assistants (CNAs) an hour to an hour and a half to answer [their] call light. The resident stated it happened mostly at night and on the weekends and it made [them] mad when it happened.”
