  • Failure to Ensure That Every Resident’s Personal Medical Records Remained Private and Confidential – citation #F164 date September 3, 2016
  • The state investigators determined that the nursing home “failed to ensure privacy during toileting for [one resident].” The investigators noted that at 1:35 PM on August 31, 2016, “there was a strong stool-like odor outside of [the resident’s] room. The door to [the resident’s] room was observed to be partially open, and the surveyor knocked and entered the room.”

    The resident’s “bed was directly observed on the right side of the room by the door. There was no privacy curtain pulled around the resident’s bed.” At that time, the resident “was lying in bed with his brief opened, and his sheet was by his feet. There was no staff member present. The resident stated he was being changed by [an Aide], but she left him like this to go get something.”

    Two minutes later, a Regional Nurse “was walking in the hallway right side of the resident’s room. She was asked who the resident’s Aide was. She inquired why, and the surveyor replied that the resident was left in the middle of a brief change, and no one had returned.”

    The nurse stated that “this was not right. She came into the resident’s room and assured him she would find the Aide to help him.”

    A few minutes later, a second interview was conducted with the resident. He stated that “he did not like that the Aide left him with his brief open. He stated that the Certified Nursing Assistants did that all the time. He stated that [the Aide] was going to clean him more, but she needed help, so she left to find help. She did come back and clean him up, but she needed help, so she left to find help.”

    The resident stated that “he was not completely clean and still needed more cleaning. The room still had a strong urine and stool odor.” The Aide, upon her return, stated that “she went to clean the resident but did not have enough towels to do so. She stated she also needed to stand him up to clean him well. She could not do that by herself.”
