  • The state investigators “determined that the facility failed to follow the implementation of delivering a gastric tube feeding for [one resident].” The surveyors observed a resident lying “flat in bed with a tube feeding flowing.” A Registered Nurse (RN) was asked if the tube feeding was on due to the feeding.” At that time, the pump was covered with a pillowcase for dignity, and it was impossible to view the pump.

    The RN stated that they were not sure, then “moved the pillowcase which revealed the feeding pump was running at 25 mL per hour and paused the pump.” The RN stated later that day that “the head of the bed appeared to be flat and the feeding pump should have been paused whenever the head of the bed was flat” to prevent medical issues.

  • Failure to Provide and Implement an Infection Protection and Control Program – citation date April 20, 2018
  • According to investigators, the facility “failed to establish and maintain an infection prevention and control program designed to provide a safe, sanitary and comfortable environment.” The facility also failed to “help prevent the development and transmission of communicable diseases and infections.”

    The state investigators observed two Certified Nursing Assistants (CNAs) who were “wearing gloves upon entrance to the room [when] equipment was already set up to provide incontinence care.” One CNA “removed trash bags from her pocket, opened the bags, and placed the open bag on the floor with gloves on. She proceeded to clean the sides of the resident’s groin area, then with the middle of the washcloth, she cleaned the front peri-area and did not change gloves,” which might have been contaminated.

  • Failure to Provide Care for Residents That Builds or Maintains Their Dignity and Respect of Individuality – citation date February 8, 2017
  • The surveyors determined “the facility failed to promote care for residents in a manner and an environment that maintained/enhances resident’s dignity and respect in full recognition of [their] individuality.” The nursing home also failed to “refrain from practices demeaning to [two residents].”

    In one incident, the facility “failed to empty [the resident’s colostomy bag promptly], resulting in the bag bursting open and spilling feces on the resident.” The resident stated that “it was so humiliating and dirty.”
