  • The state investigators determined the nursing home "neglected to protect a cognitively impaired resident from harm that resulted in multiple bruises to her mid sternum, left arm, left axilla [the armpit] and left inner thigh on March 2, 2018.” Bruises were noted “to her left arm and left axilla” the following day.

    On March 19, 2018, bruises were found “under her right breast, right axilla, right sternum, and under her right ribs that extended down to her right hip.” The state investigators determined that an Immediate Jeopardy situation began on March 2, 2018, when the resident “was noted to have bruising to her left arm, midsternal, left axilla, and left inner thigh.”

    The Staff Development Coordinator stated that she recalled the resident “having a large area of bruising on her left side.” The coordinator stated that “she did not know what caused the area [and said] she does not investigate incidents. She said [if] she suspected a resident had been abused, she would report it to the Administrator.”

    The investigators interviewed the resident’s Physician, who would call the bruising the resident had “significant in size. She said she thought the bruising had come from a witnessed fall or some type of trauma. She said the facility had informed her that the resident had not had any reports of falls.”

    The resident’s Physician stated that they wanted to order x-rays. However, the resident refused. A Nursing Assistant stated that the resident “is a two-person standard pivot [patient] with transfers. She said the resident is not combative with care and needs help with her care.”

    The facility Administrator stated that they had been in contact with the resident’s Physicians and “she had said that the bruising had come from the resident being combative with care.” The Administrator stated that they think “that the bruising had come from the resident being combative with care.” The Administrator believes that “the resident might have hit herself on the table and it had caused the bruising.”
