According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, one out of five stars for staffing issues, and two out of five stars for quality measures.

  • Failure to Report and Investigate Any Act or Reports of Abuse, Neglect or Mistreatment of Residents – citation #F225 date August 20, 2015
  • The state investigators determined that the nursing home “failed to immediately report alleged abuse [according to] State Law through established procedures for [one resident].”

    On August 18, 2015, during a resident interview, the resident “revealed to [the surveyor] that his roommate had hit him in the face and head twice in the past month or so.” The resident stated that “he had not provoked the resident in any way.” The resident’s daughter present at the interview, “revealed that the facility had contacted her about the incident, but they had not, to her knowledge, done anything more about the incident. His daughter had no knowledge of the incident being reported to any State Agency.”

    The Social Services Director stated the same day that “there had been a reported incident by [the resident and revealed that the resident] reported that his roommate had struck him in the face. She revealed that the facility completed an investigation but did not report to the Abuse Registry because the incident was not founded or validated.”

    The Director of Nursing stated that same day that the incident between the residents “was not reported to the Abuse Registry but investigated solely by the facility and found not to be substantiated. She revealed the incident was handled as a grievance,” instead of as an allegation of abuse. The Director revealed that the resident “did not have any injuries to support his allegation, so he was only offered to move rooms.”

    The surveyors interviewed the Administrator, Director of Nursing and Assistant Director of Nursing, who revealed that “all employees are responsible for reporting abuse. The three revealed that any employee that witnessed or had knowledge of abuse should report to their supervisor or directly to the Abuse Coordinator/Designee immediately.”
