  • Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Prevent Avoidable Accidents – citation #F689 date August 12, 2019
  • The state investigators determined the nursing home "failed to adequately assess [a resident] for the risk of elopement (leaving the facility unsupervised and undetected). This failure resulted in an elopement [by the resident].” The deficient practice by the nursing staff “had the potential of [causing] serious injury to the resident while out on the street.”

    The Investigation Report dated July 23, 2019, documented that [the resident] “walked from the facility to her daughter’s home, which is approximately 1 mile and through downtown San Rafael.” The Change of Condition indicated that the resident had eloped and “went home walking, leaving the facility unnoticed.” The document shows that the resident “was not in her room when [the nursing staff was] doing a medication pass for the night.”

    The facility Director of Staff Development stated that the resident “was not a risk for elopement” as indicated in the Elopement Risk Assessment and “did not trigger as a risk for elopement on the admission nor by her behavior; she did not trigger for continuing quarterly assessments.”

    In a separate summary statement of deficiencies dated November 6, 2018, the “facility failed to accurately assess [a resident] and provide adequate supervision to avoid an elopement (the resident leaving the facility unsupervised and undetected).” This failure resulted in [the resident] “leaving the facility, falling, and [being] treated at the Emergency Department for an abrasion to the left knee. This failure had the potential for the resident suffering serious or life-threatening injuries.”
