Castleton On Hudson New York Nursing Home Safety Concerns

The state of New York and Medicare.com routinely update their comprehensive list of deficiencies online to reflect all violations.
According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, three out of five stars for staffing issues and two out of five stars for quality measures.
- Failure to Provide Every Resident an Environment Free of Accident Hazards and Provide Adequate Supervision to Prevent Avoidable Accidents – citation #F689 date June 12, 2018
The state surveyors determine “the facility did not ensure that each resident received assistance devices to prevent accidents.” Specifically, for one resident, “the facility did not implement a care plan and intervention for an assistance device to be used in a wheelchair to reduce the resident’s risk of falls. This [failure] resulted in a fall from the wheelchair on May 14, 2018.”
The severely, cognitively impaired resident’s Accident & Incident Report dated March 5, 2018, noted that “the staff saw the resident slide out of the wheelchair onto the floor. No injuries were noted. A physical therapy evaluation was requested following the fall.”
Physical therapy “provided the resident with a pommel wedge cushion (a cushion with an upward-projecting protuberance at its front part to prevent the wheelchair-bound patient from slipping down on the seat).” A new Accident and Incident Report dated May 14, 2018, documented that “the resident was found on the floor in front of his wheelchair. The assessment documented a small laceration [measuring] 1.5 cm on the left eyebrow.”
An updated Comprehensive Care Plan dated May 17, 2018, noted a change in the size of the wheelchair cushion because the Rehabilitation Director stated “the resident had a cushion that was not appropriate for his wheelchair. The pommel wedge cushion was implemented after the fall.”
The Director stated that “it was not the cushion that she gave him in March. The original cushion that he had at the time of the fall in March was removed and replace with a wedge cushion.” The Director stated that “the chair is not that wide. He has a small man. She made a recommendation for the pommel wedge cushion. Nursing should follow rehab’s recommendations.”
The Assistant Director of Nursing stated that “the day before the fall in May, the resident was in the wheelchair at the Nurse’s Station. There was no cushion in his chair. She found the cushion in his room and placed the cushion under the resident.”
