- In a separate summary dated April 13, 2017, the investigator stated that the facility “failed to maintain an infection control program to provide a safe, sanitary environment for the prevention of infections.” The surveyors documented that “there was a break in a clean technique during peri-care for [a resident] and the staff did not remove soiled gloves during wound care for [another resident] and urinary catheterization for [a third resident].”

- Failure to Develop, Implement and Enforce Complete Care Plans That Meet All of the Resident’s Needs – citation date August 22, 2018
According to surveyors, the facility “failed to maintain an effective infection control program to reduce the risk of the spread of infection.” The deficient practice involved one resident who contracted Methicillin-resistant Staphylococcus aureus (MRSA – a highly contagious bacteria) infection, in the same room as [another resident, even though the second resident] had invasive devices.” According to documentation, the facility failed to place the first resident “on contact precautions until two days after admission.”
The surveyors also documented that “staff did not wear Personal Protective Equipment (PPE) in [a fourth resident’s] room that was under contact isolation” creating an environment where infection could easily spread to other residents in the facility.
The state surveyors noted that the facility “failed to ensure Physician’s orders were followed for” one resident “related to wound care. Also, [the resident] had a change in skin integrity and the Physician was not notified.” The investigators interviewed the resident who stated that they “could not remember the last time the staff changed the [wound] dressing.”
According to state surveyors, the nursing home “failed to implement the Care Plan for [two of four residents].” One resident’s “activities of daily living interventions and [another resident’s] injury risk interventions were not implemented according to the care plan.”
