According to investigators, the "facility failed to implement contact precautions for one resident." A review of the resident's Hospital History and Physical Report shows the resident had a "fracture after a fall and had placement of the boot." The resident had been taking IV antibiotics to treat a pressure ulcer for cellulitis to their left leg.
Also, the resident was "on contact precautions for Methicillin-resistant Staphylococcus aureus [a highly contagious dangerous infection] in the left ankle wound. Surveyors say that there was no order to "discontinue contact precautions" when the resident was observed "sitting in her wheelchair talking to several visitors. There was no sign on the door or equipment outside the door to indicate the resident was on contact precautions" to protect other residents, visitors and employees.
During the observation, "one visitor was sitting on the resident's unmade bed." Afterward, the surveyors observed the Licensed Practical Nurse (LPN) performing wound care on the resident when "there was no sign on the door indicated the resident was on contact precautions for any isolation gowns or cart outside the door."
The survey team interviewed the Director of Nursing who "confirmed the order for contact cautions but stated [the resident] had not been placed on contact precautions because her wound was not draining."
The nursing home "failed to conduct a thorough investigation of alleged abuse for two [residents] reviewed for abuse." Specifically, one resident "alleged, during a Care Now Services counseling session, that someone had hit him on the right side of his face." The documentation shows that the facility concluded that the resident "was just delusional without conducting a thorough investigation."
A separate incident involved another resident with an "acute fracture involving the tibia and fibula. Although the facility was unable to substantiate how the fracture occurs, the facility did not conduct a thorough investigation into this injury of unknown origin."
