According to Medicare, this facility maintains an overall rating of two out of five stars, including two out of five stars concerning health inspections, three out of five stars for staffing issues and two out of five stars for quality measures.

  • Failure to Protect Every Resident from All Forms of Abuse Including Physical and Sexual Assault, Physical Punishment, and Neglect by Anybody – citation #F600 date May 12, 2018
  • The state investigators determined that the nursing home “failed to prevent physical abuse for [one resident] reviewed for abuse. This [deficient practice resulted in a resident] receiving lacerations to the top of the forehead and left forehead, multiple bruises to the face, and multiple open bite marks to [the resident’s] right hand.”

    The investigators observed a resident just before noon on May 7, 2018, “sitting in the dining room for lunch.” The resident “has a laceration to his forehead, bruising, and scabs to his face/cheeks/eyes, and several scabbed areas to his right hand.”

    A review of the resident’s SBAR (Situation, Background, Assessment, Recommendation) dated April 26, 2018, signed by a Registered Nurse (RN) documented that the resident “was involved in a physical altercation with [another resident] [and was] sent to the local emergency room for evaluation.”

    A subsequent nursing note indicated that the resident came back from the local emergency room with “ten bite marks to the right hand, bruising to the right side of the face [that was] purple in color.” The top of the resident’s head had lacerations that were 7.0 mm in length and 1.0 mm wide with “a small bump in the middle of the laceration to the head.”

    A Certified Nursing Assistant (CNA) stated that “[an RN] and I heard a noise by [the resident’s] room, we tried to open the door, but could only crack it.” At that time, “[one resident] was on top of [the other resident] blocking the door. We entered through the other door and pulled [one resident off of the other resident].”

    The CNA stated that they were “at the nurse’s desk and heard a thump at [the resident’s] room.” The CNA “saw blood all over [one resident].”
