The state investigators determined the nursing home "failed to administer a resident’s [opioid medication] on the skin patch as ordered by the Physician and notify the Physician of the missed medication doses on March 22, 2019, and April 24, 2019.” The deficient practice “resulted in ineffective pain management for [the resident].”
The investigative team reviewed the resident’s Care Plan dated February 13, 2019, indicating that the resident had “a history of frequently moderate and severe pain related to [their] back and wound indicated interventions that included administering pain medication as ordered.”
However, the facility did not have the medication for a scheduled dose. The resident stated that he was without the medication patch to treat his pain “for five days and was in a lot of pain.” A Licensed Vocational Nurse said that the resident’s medication “was not available on his scheduled due date on March 22, 2019, and the pharmacy delivered [the medication] on March 24, 2019.”
In a separate summary statement of deficiencies dated October 12, 2018, the facility “failed to properly assess and manage pain for [one resident]. This deficient practice resulted in [the resident] experiencing distress due to severe and mild pain.”
The survey team reviewed the resident’s History and Physical and Nursing Care Plan, indicating that the resident “had bilateral sores on her tongue.” On August 27, 2018, the resident had “complained of pain [measuring ten out of ten] due to mouth sores.” The resident “stuck out her tongue to show a mouth ulcer on the left side of her tongue.”
The resident stated that “she had been feeling miserable for many months and stated that all the nurses know about her pain, but they did not care, and they did not say anything.” The resident told the surveyors that she experienced pain at a level ten where “ten is the worst imaginable pain.”
A Certified Nursing Assistant providing the resident care stated that she had been working with the patient for the last month, and during that time, the resident had complained of pain in her mouth.
