According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, two out of five stars for staffing issues and two out of five stars for quality measures.

  • Failure to Protect Every Resident from All Forms of Abuse Including Physical and Sexual Assault, Physical Punishment, and Neglect by Anybody – citation #F600 date February 23, 2018
  • The state investigators determined that the nursing home “failed to prevent the physical abuse of [two residents] and verbal abuse of [a third resident] by employees of the facility.”

    The state investigators reviewed a document titled: Final Report documented on January 12, 2018.” The report indicates that an allegation of abuse occurred. It was reported in January 2018, by a Licensed Practical Nurse (LPN), that they had witnessed a Certified Nursing Assistant/Hall Assistant sitting on a resident “in an attempt to calm and keep [the resident] from exiting the building.”

    A witness statement signed by the LPN documents that they had walked past the resident’s room when the resident was “sitting on the edge of the bed and [the CNA] was sitting on top of [a resident].” The CNA then walked out of the resident’s room and “closed the door, holding the handle, so [a resident] couldn’t leave the room.”

    The LPN then talked to the Charge Nurse and told that nurse that “we cannot sit on residents,” and the Charge Nurse stated, “I told [the CNA to sit on the resident].” The Charge Nurse stated in a document that they were “at wit's end and [they] did not know what else to do.” The LPN stated “I am not a psyche (sic) nurse and really did not know what to do.”

    The CNA stated that they had sat on a resident’s lap while the resident sat on the edge of the bed. The CNA stated that the resident had been trying to exit the building and that the Charge Nurse on Duty told the CNA to sit on the resident. The CNA also stated that the door was held by their foot and hand so the resident could not get out of the room once the CNA had gotten off the resident’s lap.
