The state investigators determined that the nursing home “failed to timely report an allegation of neglect to the Alabama State Survey Agency. This deficient practice affected [one resident] involved in allegations of abuse or neglect.”
The state investigators determined that “the facility submitted to the Alabama State Survey Agency (Alabama Department of Public Health, DHCF) an allegation of neglect. According to the report, [a Registered Nurse (RN)] failed to perform cardiopulmonary resuscitation (CPR) on [a full code status resident] who was found unresponsive.”
The facility Administrator verified the date of the “failure of the licensed staff member to initiate CPR on the resident [who had] full code status.” The Administrator also confirmed that the incident “should have been reported to the Alabama State Survey Agency [within] two hours of the incident.” The Administrator stated that “she did not know it was a reportable event that should have been submitted to the Alabama State Survey Agency.”
The investigators asked, “what made the event a reportable incident.” The Administrator responded, “Neglect.” The surveyors determined that the facility failed to “provide basic life support, including CPR, prior to the arrival of emergency medical personnel, subject to Physician’s orders and the resident’s advance directives.”
The CNA plus had entered the resident’s room “and found the resident unresponsive.” The CNA informed a Registered Nurse (RN) who “was certified in CPR.” The Registered Nurse “assessed the resident and found [a resident] did not have a pulse or respirations. After the assessment, [the RN] left the resident’s room to activate the emergency response system [and] did not return to the resident’s room to initiate CPR.”
The surveyors reviewed the Ambulance Patient Care Report that indicated that “CPR was not being performed when they arrived.” It was determined that “this deficient practice placed [this resident] in Immediate Jeopardy of serious injury, harm, or death.”
