The state investigators determined the nursing home "failed to ensure that [one resident] receives necessary treatment and services to prevent a new pressure ulcer (localized damage to the skin or underlying tissue that usually occurs over a bony prominence as a result of pressure or pressure in combination with shear/friction) from developing after she had a pressure ulcer on her buttocks.”
The facility had removed the resident “from the bed with the low air loss mattress and replaced it with a regular facility mattress. While on the regular facility mattress, from October 13, 2018, until October 29, 2018, [the resident] developed a DTI (deep tissue injury) pressure ulcer on her sacrum (tailbone).” These “failures contributed to the development of a deep tissue injury to [the resident’s] sacrum and had the potential to cause further skin breakdown to this resident.”
The “facility failed to provide smoking safety assessments on each individual resident’s assessed needs, adequate monitoring, and safety equipment to ensure the safety of [three residents] who smoked.”
In one incident, the resident “was not assessed for smoking safety [for some time] and was observed smoking in an undesignated smoking area without using safety equipment (smoking apron), managing his cigarette supplies, and did not dispose of cigarettes in the proper smoking receptacle.” Another resident “had no smoking screen assessments done, was not wearing smoking safety equipment (smoking apron), was not in the designated smoking area, and did not use the appropriate trash receptacles when disposing of cigarettes.”
A third resident “was smoking with a smoking apron, in an undesignated smoking area, and did not use the appropriate smoking receptacles consistently.” It was determined that “these failures have the potential for [three residents] to be at risk for smoking-related injuries. This failure to not ensure residents had a current and accurate smoking assessment and were smoking in an undesignated area without smoking equipment placed all residents, staff, and visitors at risk for fire-related injuries.” The state investigators declared an Immediate Jeopardy situation.
