The state investigators determined that the nursing home “failed to ensure [two residents] reviewed received adequate supervision to prevent accidents.” In one incident, a review of the resident’s medical records revealed that this resident “had an unwitnessed fall on December 3, 2017, with documentation that no injuries occurred.”
While the nursing staff notified the family and Physician that the resident had a fall, they received no new order. “Fall intervention care plans were in place.” The resident revealed that “she had symptoms of increasing pain to her left wrist/arm [as evidenced] by grimacing and guarded movements. There was no documentation of the fall or observed injury.”
The agency hotline received an electronically sent notification on December 11, 2017, with “no proof that the report had been reviewed by the agency.” An interview with the Director of Nursing “confirmed that she had not followed up with the agency to see if the report had been accepted for further investigation.”
The facility Administrator confirmed that “[the Assistant Director of Nursing] is appointed as the Risk Manager” and “should have been responsible to complete the investigation, not [the Director of Nursing].” The Director of Nursing confirmed that “she had not picked up a copy of the police report from the local police department as of December 30, 2017, to determine the outcome of the investigation of an injury of unknown origin for [the resident].”
A Certified Nursing Assistant (CNA) stated that “she had been assisting another resident when she came out of the room, and she noticed that [the resident’s] call light was on.” The CNA answered the call light, noticed the resident on the bedside commode with blood on the floor and the resident’s legs . The CNA said they “did not remember if [the resident] said she fell.”
