According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, three out of five stars for staffing issues, and three out of five stars for quality measures.

The state investigators determined that the nursing home “failed to ensure [one resident] with pressure sores received the services needed to promote the prevention of a pressure sore.” The state surveyors began an investigation after interviewing the resident who “was asked if he had any concerns regarding his care.”
The resident responded, “yes, I always have a bowel movement at night, normally after or before midnight, and just last weekend, I called for assistance after having a bowel movement at 12:15 AM.” The resident stated that they “did not send a Certified Nursing Assistant (CNA) until 1:35 AM. Other times have occurred needing cleaning after a bowel movement, and I have had to wait for one to two hours for care to be cleaned.”
The resident stated, “I used to keep track of the dates and the time it took them to respond, [but] I gave up on them. I had a bad wound on my buttocks over a year ago, and I am afraid to have another open area of a pressure ulcer to happen again.”
The nursing home “failed to prevent the possible spread of infection by staff not completing hand hygiene, not providing a barrier for wound care supplies, and not cleaning scissors before use when conducting wound care for [two residents].”
The state surveyors observed a Registered Nurse (RN) providing a resident wound care. The nurse “washed her hands, applied gloves, assisted [the resident] to her right side, and pulled on the bedcovers. The RN removed her gloves, did not wash/sanitize her hands, applied gloves and removed a soft boot from [the resident’s] right foot, removed scissors from her pocket, and cut the dressing wrapped around [the resident’s] right foot/ankle.”
The Registered Nurse then placed the scissors on the air mattress and “without changing gloves, washing or sanitizing her hands, and [removed] a dressing from [the resident’s] right lateral foot.” The RN continually failed to follow established protocols when maintaining a sterile environment while providing wound care.
