    The facility “failed to notify the resident’s Physician of a need to alter treatment when the resident began and having two to three bowel movements per day for the past month and had three loose bowel movement since breakfast that morning.” The investigators interviewed and Licensed Practical Nurse (LPN) to reveal “the night shift nurse was required to review all resident output records,” and the resident’s “Medical Doctor should have been notified that the resident was having two to three bowel movements per day.”

  • Failure to Provide Safe and Appropriate Respiratory Care for Resident When Needed – citation date June 7, 2018
  • The investigative team documented that the nursing facility “failed to provide respiratory care consistent with Physician’s orders.” The resident “had a Physician’s order for oxygen at 4 Liters. The facility failed to ensure oxygen was provided to the resident.”

  • Failure to Provide and Implement an Infection Protection and Control Program – citation date June 7, 2018
  • The surveyors noted that the facility “failed to ensure and maintain an infection prevention and control program for” one resident receiving incontinence care. Documentation shows that the facility “failed to utilize proper hand hygiene and infection control.”

  • Failure to Ensure Residents Receive Proper Treatment to Prevent New Bedsores or Heal Existing Pressure Sores – citation date April 20, 2017
  • The surveyors noted that the “facility failed to ensure a resident having pressure sores received adequate care to promote healing and prevent infection.” The state surveyors noted that during an observation of wound care, the nursing staff did not clean the resident’s wound “per Physician’s orders.”

  • Failure to Give Proper Treatment to Residents with Feeding Tubes – citation date April 20, 2017
  • The surveyors documented that the nursing home “failed to ensure residents with gastronomy tube’s received appropriate treatment and services to prevent complications.” According to the documentation, a Registered Nurse (RN) providing the resident care “failed to verify placement before administration of medications. Also, [the RN] administered to medications to [the resident]. However, the nurse failed to flush between medications,” as required by established standards of care.
