According to Medicare, this facility maintains an overall rating of two out of five stars, including two out of five stars concerning health inspections, two out of five stars for staffing issues and four out of five stars for quality measures.

  • Failure to Protect Every Resident from All Forms of Abuse Including Physical and Sexual Assault, Physical Punishment, and Neglect by Anybody – citation #F600 date May 16, 2018
  • The state investigators determined that the nursing home “failed to ensure [one resident] remained free from physical abuse when [a staff member] argued with [the resident] and grabbed [the resident’s] arms, causing a large skin tear and bruising on the left arm. [The resident] was sent to the emergency room for treatment.”

    The investigative team reviewed the resident’s Nurse’s Notes dated April 26, 2018 at 3:11 PM.” The document reveals that three days earlier on April 23, 2018, the Assistant Director of Nursing “overheard [the resident] hollering out and went to his/her room. Upon entering the room, he/she noted [a Certified Nursing Assistant (CNA)] standing across from [the resident] who was also standing, and [the CNA] had been holding and was holding [the resident around the resident’s] wrists.”

    The Assistant Director of Nursing stated that they asked what was going on and the CNA and the resident “were both talking loudly at the same time.” The Assistant Director asked the CNA “to step out of the room so [the resident] could calm down and tell the Assistant Director what happened.” The resident said the CNA “got upset because he/she wanted his/her breakfast heated. [The resident] had a very large skin tear involving what appeared to be fat and muscle tissue on his/her left forearm.”

    The nursing staff noted a bruised and swollen area on the left forearm and sent the resident “to the emergency room for evaluation.” The resident indicated during an interview that “someone twisted his/her arm [and] he/she had bruises on his/her right forearm which were adjacent to the approximated skin tear and a bruise to his/her left hand.”
