According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, two out of five stars for staffing issues, and four out of five stars for quality measures.

  • The state investigators determined the nursing home "failed to provide an environment free from mental abuse for [a resident] when a Certified Nursing Assistant (CNA), while providing care to the resident, used a cellular telephone to record a video of [the resident] showing his genital area.”

    Additionally, the CNA “made inappropriate comments related to the video and distributed the video through social media. This failure resulted in mental abuse to [the resident] by the CNA.”

    The state investigators made an unannounced visit to investigate the facility-reported incident regarding a short video clip of the resident “with an exposed genital area that was posted to social media by [the CNA].” The Director of Nursing revealed that “she received a telephone call from a non-employee of the facility. The person told the Director that he received a copy of a video clip sent by a friend of one of the facility staff (later identified as the CNA) via Snapchat.” The Administrator had received a copy of the video at the same time.

    The Administrator stated that the CNA said that “she thought it was the wrong thing to do and [it] should not have been done.” The survey team observed the Snapchat video clip in the presence of the Administrator. The CNA’s face was revealed at the beginning of the video clip, and words were heard “you think we have it easy? This is what we have to deal with every [***]ing day. The second scene of approximately five seconds long showed [the resident] lying in bed with a shirt on but without pants.”

    This portion of the video clip shows the resident’s “left leg crossed over his bent right knee, with the genital area exposed. The scene included a close-up view of about two seconds of the resident’s genital area [and the resident’s] face was seen from a distant view on the video.”

    The third scene of approximately “two seconds long showed the faces of [the CNA and a Licensed Vocational Nurse] laughing at the scene that was just taken on video.”
