  • Failure to Report and Investigate Any Act or Reports of Abuse, Neglect or Mistreatment of Residents – citation #F225 date June 6, 2017
  • The state investigators determined the nursing home "failed to report an incident of alleged abuse for [one resident] to the State Agency and Adult Protective Services within twenty-four hours. The facility did not report the results of the investigation within five working days from the date of the incident (or knowledge of the incident). This deficient practice had the potential to place residents at risk for harm or mistreatment.”

    The Social Service Designee stated that a resident “came to his office on April 11, 2017 and reported an incident of alleged abuse that happened [the day before]. The Social Service Designee stated that he then completed a witness statement and reported the incident of the alleged abuse to the Administrator.”

    The Administrator stated that “he did not realize he needed to report all allegations, even if the facility found they were unsubstantiated.” The Administrator said that “the ombudsman had recently informed him that he needed to report all alleged incidents of abuse.”

  • Failure to Ensure the Nursing Home Area Remained Free of Accident Hazards and Risks and Provides Supervision to Prevent Avoidable Accidents – citation #F323 date May 19, 2017
  • The facility “failed to maintain the safety of the residents by [their] failure to post ‘No Smoking Signs’ outside the resident’s room while [the resident] was receiving oxygen.”

    The nursing home also failed to “supervise and account for the whereabouts of [a resident] who was able to leave the facility without staff knowing. This deficient practice placed the resident at risk for accidents, environmental hazards, or unsafe wandering. These deficient practices had the potential to place residents at risk for injury or harm.”

    The investigative team reviewed a Progress Note indicating that one resident “left the facility at 4:30 PM without informing staff. The resident was searched for by the staff and numerous attempts to contact the resident were made. The resident came back to the facility on the same day at 6:15 PM.” The resident stated that he had gone next door to the church “to pray for his father and fell asleep.”
