According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, two out of five stars for staffing issues, and three out of five stars for quality measures.

  • The state investigators determined that the nursing home “failed to ensure residents were free from neglect and abuse when [one resident] was not offered assistance with toileting. Instead, [the resident] was told to urinate in her underwear and was not offered a bedpan or commode. The deficient practice resulted in [the resident’s] discomfort and feeling awful sitting in a wet diaper.”

    The state investigators interviewed the resident “in the presence of the Director of Nursing, Quality Assurance Nurse, and Regional Nurse Consultant.” The resident stated “I wet the pads because they do not come on time (the staff), they, [the staff], say do it (urinate) in diapers. All night especially, I wear diapers. It is not the same when you know you can do it (urinate in the bathroom). I do not want the feeling of being wet. I feel awful because I have to go in the diapers, wet the diaper all night.”

    The resident told the surveyors that “it happens not only to her but also her roommate.” The resident stated that “when she [leaves] (the staff member leaves at the end of the shift), that is the only time she changes us. It happens all the time, even in the morning.” The investigators checked the Skilled Daily Nurse’s Notes with a checkmark of when a diaper or pad was utilized. The record indicates that the resident had used diapers or pads on every consecutive day between July 22, 2018, and July 31, 2018.

    The Quality Assurance Nurse stated that “if the resident is able to use the bedpan or commode, the staff should not let them use a diaper.” The nurse also said that “management will investigate the incident.”
