The state of Delaware and the federal government regularly updates its long-term care home database system with complete details of all deficiencies, citations, and violations.
According to Medicare, this facility maintains an overall rating of two out of five stars, including one out of five stars concerning health inspections, four out of five stars for staffing issues and two out of five stars for quality measures.

The state investigators determined that “the facility failed to identify an allegation of neglect. In addition, the facility failed to identify an allegation of mistreatment for [one resident].”
These failures “resulted in a lack of immediate reporting to the State survey agency in accordance with State law, lack of thorough investigation, and a lack of protecting the residents.” The surveyors reviewed the facility’s records and investigation files along with the written statement addressed to the Assistant Director of Nursing.
The document states that the resident was observed by a Certified Nursing Assistant (CNA) “lying in a bowel movement at the beginning of the 7:00 AM to 3:00 PM shift.” A Response to Education Request Form was directed to the CNA who was assigned to provide care to the resident during the previous shift (11:00 PM to 7:00 AM). The area of concern for education included resident and patient rounds.
The document indicates that the last form was not completed for the resident “and to please check all of your resident/patients during the last rounds to ensure that the resident was clean and dry.” A facility Registered Nurse (RN) advised the Director of Nursing that the education was warranted “to ensure competent care was completed.”
The Registered Nurse confirmed that “the facility failed to identify this incident as an allegation of neglect. This failure resulted in a lack of immediate reporting, lack of investigation, and a lack of protection of residents.” The oncoming CNA documented that the resident “was found lying in his bed this morning with brown urine stains under him with towels and bed.” The oncoming CNA stated, “I guess [the towels were] to soak it up.”
The staff confirmed that “the facility failed to identify this incident as an allegation of neglect.” A different CNA documented that on several occasions, the same resident “had been found in bed with clothing that was saturated in urine.” The resident’s “bed linens were soaking wet, and they had brown urine stains.” A fourth CNA documented that “approximately four to five months ago, [they] came into work and found [the same resident] with the same clothing that he on the previous day and [the resident] was soaked with urine from his shirt to his jeans.”
