  • The state investigators determined that the nursing home failed to ensure two residents “were free from abuse perpetrated by [a third resident] who resides on the Homestead Memory Care (Dementia) Unit.”

    On January 25, 2019, at 3:05 PM, the resident was observed “ambulating in the hallway and bleeding from injuries to the right hand. There were three lacerations across the resident’s knuckles. The trail of blood was followed, and the staff observed [the resident] in his/her wheelchair bleeding from the lower lip, with a possible broken tooth.”

    At 7:45 PM the same day, the injured resident and another resident “ambulated in the hallway toward each other, when [the injured resident struck the other resident] with his/her right fist on [the second resident’s] chin. The facility concluded [the first injured resident was] identified as being physically aggressive toward others, [and he/she] struck [the second resident] with no warning or provocation and physical abuse did occur.”

    The facility provided one-on-one supervision/oversight during the resident’s waking hours starting on February 15, 2019, until February 21, 2019. However, “during the evening shift on February 20, 2019, around suppertime, the intervention of one-on-one supervision/oversight was not implemented and [the aggressive resident] was found by staff standing over [the previously assaulted resident, punching them] in the head.”

    The deficient practice “[affected two assaulted residents] and was reviewed for the resident to resident altercations and placed [one injured resident] in Immediate Jeopardy for serious injury, harm, or death.” On March 1, 2019, at 3:40 PM, the state investigators notified the facility’s Administrator (Center Executive Director), the Director of Nursing Service (Center Nurse Executive), and Director of Clinical Services of the “findings of Immediate Jeopardy in the area of freedom from abuse, neglect, and exploitation.”
