  • The state investigators determined that the nursing home “failed to ensure [one resident] was free from verbal abuse and physical mishandling during care.” The investigators reviewed a State Agency Notification Form dated May 10, 2019. The document indicates that the resident had stated “a staff person was abusive to [them] on that date.”

    The form indicates that a Certified Nursing Assistant (CNA) “was suspended and an investigation was initiated.” The facility documents, including timecards, verified that the CNA had worked on that date. The Administrator/Abuse Coordinator reported “the allegation is still under investigation.”

    A Licensed Practical Nurse (LPN) stated that the resident had made a claim that the CNA was helping [another CNA] clean him up when [the abusive CNA] threw a urine-soaked gown in his face.” The resident then said that the abusive CNA rolled him and yanked him on his bad arm, causing pain.

    When the resident told the CNA that it hurt, the resident says that the CNA “said ‘there is not a damn thing wrong with your arm.’” The resident asked the CNA to watch their language, and the CNA said that they can say what they want because they are “grown.”

    The resident said that the CNA repeatedly called the resident “a liar and told him nobody will believe you anyway because you always lie.” The reporting CNA “was present the whole time.” It was also documented that the third CNA entered the resident’s room near the end of the resident’s care and was a witness to what was occurring.

    The resident stated that they had reported that a CNA had yanked on their left arm and hit him on the left shoulder, calling him a liar. However, the resident “did not recall the CNA’s name or the date/time of the reported incident.” The resident stated “it is all wrote up. I think [the Administrator] took care of it.”
