According to investigators, the facility “failed to provide the necessary care and services to attain or maintain the highest practicable physical, mental, and psychosocial well-being.” The nursing facility “also failed to develop a Comprehensive Plan of Care to address and control the presence of pain for [two residents].”
A review of the resident’s MDS (Minimum Data Set) Assessment revealed that the patient “had frequent pain presence, rating [their] pain intensity a six out of ten with ten being the most severe pain.” However, a review of the Care Plan revealed: “no documented evidence that the Plan of Care to address the resident’s pain had been developed.”
The state survey team determined “the facility failed to ensure the environment remain as free from accident hazards as possible.” nine residents were identified by the facility to wander and two other residents shared a bathroom.
During a tour at the facility, the surveyors observed “an open to a back on the floor which contained a box cutter, a hammer, pliers, wire cutters, and two flat-blade screwdrivers left unsecured and in plain view. In addition, observations made during the initial tour revealed in an unlocked and unattended shower room on the A Unit, which contained easily accessible personal care items that could be harmful within reach.”
