According to Medicare, this facility maintains an overall rating of two out of five stars, including two out of five stars concerning health inspections, four out of five stars for staffing issues and three out of five stars for quality measures.

  • Failure to Develop, Implement and Enforce Policies That Prevent Mistreatment, Neglect or Abuse of Residents – citation #F226 date November 7, 2016
  • The state investigators determined that the nursing home “failed to report an allegation of physical and mental abuse for [one resident] to the State Survey and Certification Agency.” The nursing home also “failed to protect [the resident] from the alleged perpetrator after the facility was aware of [the resident’s] allegation.”

    The investigation involved a cognitively intact resident who was “currently in hospice services and plans on being a long-term placement due to his/her medical condition diagnosis.” Documentation shows the resident had “potential risk for falls: use a gait belt (a special belt placed around the resident’s waist to provide a handle during transfers, ambulation, or repositioning residents in the chair) with all transfers.”

    The investigators interviewed the resident who stated that “most of the staff are okay.” However, the resident indicated that one particular Certified Nursing Assistant (CNA) “had been rough with him/her and hurt his/her arm; he/she had to go to the bathroom; [the CNA] grabbed his/her arm and was rough.”

    The resident stated that they do not exactly remember the day that it happened but that it scared him or her when the “CNA pulled on his/her left arm during a transfer.” The resident stated that the CNA had said to the resident, “that is not mean, you do not know what mean is.”

    The resident indicated that the CNA “was back in [the resident’s] room a couple of nights ago.” At a subsequent interview, the resident stated he/she “does not know exactly what day the incident occurred [but that] it occurred during the night at approximately 2:00 AM [when he/she] needed help.”

    The resident stated that the CNA had been swinging him or her around and “started ripping clothes off of him/her.”
