The state investigators determined that the nursing home “failed to ensure the interventions for fall prevention were consistently being implemented for [the resident]. This failure increased the risk of falls for [a resident].” The state investigators conducted an unannounced visit at the facility to “investigate a ‘quality of care’ issue.”
The investigators reviewed the resident’s Fall Risk Evaluation indicating a score of eighteen, meaning “high risk for fall.” The resident’s Care Plan for Falls indicated that “there was a scheduled toilet visit before and after meals.” The Clear Directive Record indicated not to leave the resident unattended in the bathroom or dining areas.
There was a toileting entry in the medical records at 9:36 AM. “There was no other documentation indicating toileting assistance was provided [that day according to] the Plan of Care. There is no documented evidence indicating scheduled toileting was conducted since the admission except [on one occasion].”
A Nurse’s Note documented at 6:30 PM indicated that a Licensed Nurse “was at the nurse’s station when ‘I heard a loud thud’ from the 200 Hallway. I heard the resident in 202 calling out. She was lying next to her bed, supine with her head against the sliding glass door. I asked the resident what happened. She said ‘I fell. I hit my head.’ She complained of pain to her head and left hip.”
An interview with the Director of Staff Development verified that there was a toileting scheduled before and after meals and at night as an intervention to prevent the resident from falling. The resident stated that they “want to go to the toilet all the time, and this could be the reason for the resident’s agitation.” The Director verified that “there was one documentation in the activities of daily living report indicating toileting assistance was provided for [the resident].”
The Director of Nursing stated that “they should be documenting in the medical record and indicating that the scheduled toileting before and after meals was being implemented.” The Director stated that “the documentation could assist the facility in re-evaluating whether the Care Plan for Falls is effective or not.”
