The state investigators determined that “the facility failed to ensure hand hygiene procedures were followed by staff involved in direct resident contact.” Specifically, one Certified Nursing Assistant (CNA) “did not wash or sanitize her hands after perineal care and before leaving [the resident’s] room to obtain linens.”
Also, a Registered Nurse (RN) “did not wash or sanitize his hands after perineal care and before setting up [another resident’s] oxygen concentrator. This deficient practice could affect three residents who are incontinent on the 600 Hall and two residents who received oxygen on the 600 Hall and could result in the spread of disease.”

The facility “failed to ensure [a CNA] demonstrated competency while providing incontinent care to [one resident] observed for incontinent care.” Specifically, the CNA did not clean the resident’s “right and left buttocks during incontinence care. This deficient practice could affect fourteen residents on the 300 Hall who received incontinent care and could result in the spread of infection.”
The state investigators observed two staff members, including one CNA providing resident incontinent care. After cleaning the resident’s rectal area, the CNA “did not clean the resident’s right or left buttocks.”
The CNA confirmed during an interview that “she forgot to clean the resident’s right and left buttocks after cleaning the rectal area.” The Director of Nursing revealed that the resident’s right and left buttock should have been cleaned after cleaning the rectal area.” The nursing staff failed to follow the facility’s proper procedures when providing perineal care that could have affected fourteen residents “who are incontinent on the 300 Hall.”
