According to Medicare, this facility maintains an overall rating of one out of five stars, including one out of five stars concerning health inspections, one out of five stars for staffing issues and one out of five stars for quality measures.

  • Failure to Develop, Write and Use Policies and Forbid Mistreatment, Neglect or Abuse of Residents – citation #F224 date October 10, 2017
  • The state investigative team determined the facility “failed to ensure [one resident] reviewed for neglect was free from neglect.” Specifically, a Certified Nursing Assistant (CNA) failed to protect the resident “by not following the Care Plan while providing incontinent care.”

    As a result, the resident “fell and sustained a laceration on the left eyebrow line and a left frontal scalp hematoma.” The resident “experienced pain as a result of the fall.”

    The CNA provided incontinent care with the resident “without assistance when [the resident’s] Care Plan specified two-person assist during bed mobility, transfer, and toilet use. This failure affected one resident and placed nine additional dependent residents at risk for falls, injury, hospitalization, pain, and neglect.”

    The severely, cognitively impaired resident “fell off the bed when [a CNA] turned the resident to the left during incontinent care. She stated the bed was locked but moved when she leaned on the bed.” The CNA stated that the resident “was care planned for total care and two-person assist with bed mobility, transfers, dressing, and bathing.”

    The survey team interviewed a Licensed Vocational Nurse who stated after she assessed the resident “and applied a pressure dressing, she gave [the resident] pain medication because she was in pain. Then she notified [the resident’s] doctor who gave an order to transfer [the resident] to the emergency room.”
