    The investigators determined that “the facility failed to conduct criminal history checks to ensure the staff member was employable. These failures could place residents in the facility at risk for potential abuse, neglect, or misappropriation.”

  • Failure to Develop, Write and Use Policies and Forbid Mistreatment, Neglect or Abuse of Residents – citation #F224 date October 14, 2017
  • The nursing home “failed to ensure each resident was free from neglect for [three residents] reviewed for neglect. The facility failed to prevent the development or worsening of facility-acquired pressure ulcers for [one resident] who was identified at risk to develop pressure sores.”

    The facility “failed to implement a Plan of Care when he was identified at risk to develop pressure sores.” The resident “developed a facility-acquired pressure sore that was first identified as a [significantly progressing] Stage III to his left buttocks. The pressure sore advanced to a Stage IV [life-threatening] pressure sore.”

    It was determined that “the facility failed to follow the doctor recommendations to offload the resident’s Stage IV pressure ulcer [and] failed to provide the appropriate treatment and implement preventive measures to avoid the deterioration of [the resident’s] Stage IV pressure sore.”

    The resident “was found with a Deep Tissue Injury on the outside of his Stage IV pressure sore, foul odor on the wound, and a large reddened area covering most of his left buttocks.”
