  • Failure to Develop, Write and Use Policies and Forbid Mistreatment, Neglect or Abuse of Residents – citation #F224 August 3, 2017
  • The state investigators determined that the nursing home “failed to ensure each resident was free from neglect, for one resident] reviewed for neglect.” Specifically, “the facility failed to provide and administer [the resident’s anticonvulsant medication every twelve hours].”

    This failure could “place all eleven residents who are prescribed anticonvulsant medications for [a disorder] at risk for neglect.” The state investigators reviewed the resident’s Medication Administration Records that revealed that there was no discontinuation order for the medication.

    As a result of the administration of the medication being missed twice daily, the resident was “transferred to the emergency room” with complications. Nearly six weeks after the resident was found unresponsive, the resident’s daughter stated that the resident “was still in the intensive care unit on a ventilator. The daughter stated that [the resident’s neurologist is the only provider who changes her anticonvulsant orders and [the resident] had not had an appointment with a neurologist to discontinue [the medication].”

  • Failure to Report and Investigate Any Act or Reports of Abuse, Neglect or Mistreatment of Residents – citation #F225 date March 31, 2017
  • The facility “failed to ensure the written investigation was submitted to the State Survey Agency within five working days of the incident” as required by law. It was determined that “this failure could place the census of eighty-five residents at risk for abuse, exploitation, and the misappropriation of property.”

    A review of the Incident Report involving two residents “was submitted by the facility to the State Survey Agency regarding a resident-to-resident altercation” where the residents were yelling at each other and needed to be separated. Even with staff intervention, both residents “managed to strike each other on the other’s shoulders.”
